How peptide therapy actually works
Peptides amplify signals your body already sends. Here's what the four families do, what the evidence actually supports, and how a prescribed protocol runs start to finish.
Peptides are short chains of amino acids — smaller than proteins, larger than single amino acids. Therapeutically, they work by binding receptors your body already uses, which is why a peptide protocol is less like taking a drug and more like amplifying a signal your body already sends.
The four families you'll actually be offered
| Family | Examples | What they're prescribed for |
|---|---|---|
| GLP-1 agonists | Semaglutide, tirzepatide | Appetite regulation, weight loss, metabolic health |
| Growth-hormone secretagogues | Sermorelin, CJC-1295, Ipamorelin | Stimulate your own GH release |
| Repair & recovery | BPC-157, TB-500 | Tissue and gut repair (mostly preclinical evidence) |
| Longevity / cognitive | Epitalon, NAD+, Semax, GHK-Cu | Cellular aging, cognition, skin |
The evidence is not equal across families
GLP-1s have large randomized human trials behind them. BPC-157, epitalon, and several others rest
overwhelmingly on animal data with thin human evidence. Anyone who tells you the whole category
is equally proven is selling something. Read the compound-level science at
PeptideOnline. How a prescribed protocol actually runs
- Intake — health history, contraindication screening, goals.
- Clinician review — a licensed prescriber decides if you're a candidate.
- Compounding — a 503A pharmacy prepares your patient-specific prescription.
- Titration — you start low and step up over weeks, especially on GLP-1s.
- Monitoring — ideally bloodwork; in practice, only some providers require it.
FAQ
Do peptides actually work?
It depends entirely on which peptide. GLP-1 agonists have large randomized human trials (STEP-1: ~14.9% body weight loss over 68 weeks). BPC-157 and several longevity peptides rest mostly on animal data with very limited human evidence. Judge each compound on its own record.
Are peptides steroids?
No. Peptides are short amino-acid chains that signal through receptors; anabolic steroids are synthetic hormones. Some growth-hormone secretagogues are on the WADA prohibited list for competitive athletes, which is a separate issue from legality.
How long until peptides work?
GLP-1 appetite effects often appear within 1–2 weeks; meaningful weight loss takes 8–16 weeks. Recovery and longevity peptides are typically run in cycles of 4–12 weeks, with much less reliable evidence about what to expect.
